Enrollment

Upon enrollment of your child, we will give you a copy of our packet but if you would like to view our packet, it is listed below:

You may print, fill out, and submit the paperwork for your child's enrollment to our center. Along with the packet, we need a copy of your child's immunization record (an example is in the PDF).
